I certify that I am the owner of the computer or device being presented for technical service. I understand that no verbal or written warranty, expressed or implied, is offered on the services rendered by Cypac. By accepting these services I agree that Cypac cannot be held responsible for any damages to hardware or software resulting from this service. Furthermore, Cypac will not be liable for future damages or hardships resulting from any part of the services provided. I give authorization to the Seress, LLC dba Cypac ("Cypac") staff to open my computer for servicing. I understand that the Cypac staff members may not be manufacturer certified service technicians. I agree to release, indemnify and hold harmless any agent of Cypac from liability for any claims or damages of any kind that may arise from servicing my computer or device. I also understand that the hardware is my property and that Cypac is not responsible for its maintenance or repair. I understand that the backup of any necessary data is my responsibility. Cypac will not be held responsible for the loss or corruption of any data or application resulting from services rendered. I expressly waive all claims against Cypac for any damages to the hardware or software of this computer that are incidental to the technical service rendered by Cypac. I hereby authorize Cypac to perform these services. In consideration for the services provided by Cypac, I understand and agree to the terms and conditions stated above. *
